I have no previous experience that would cause me concern about my pet's behavior as it relates specifically to the Curbside Check-in handover of my pet to the CCAH technician.(Required)
I understand there is an inherent risk involved during the handover of my pet during the Curbside Check-in process and acknowledge the CCAH staff will take every precaution possible regarding the safety of my pet. CCAH shall not be liable for any accident, injury, or death that may occur from the Curbside Check-in handover.(Required)
I will be available at the phone number provided during my pet's entire appointment. I understand that I need to be available to discuss any findings or treatment options. If I do not answer my phone, I understand that this may extend the duration of my pet's visit until I can be reached.(Required)
I understand I must wear a mask covering my nose and mouth and that I am responsible for getting my pet out of my vehicle. I understand that my pet must be on a leash or in a carrier.(Required)
I agree to pay for all services at the time they are provided. I agree to pay for administrative fees charged due to any late payments. Invoices that are not paid within 90 days will be turned over to a collections agency absent a pre-visit arrangement with CCAH.(Required)
